One web host that I used to use is xenweb.net, a free forum-based host that should offer you what you need and has cpanel with PHP, MySQL, etc…

You may check free web hosting from freehostia , not sure whether it would meet your criteria , you may also find review of top ten web hosting services at website like .

I’ll host a MySQL server for free for you on my own server for a while if you want.
No cpanel but if you download the MySQL administrator software you can use that to manage the MySQL database(s)
